GENERAL
This SIRCA investigation is to determine the different route causes of an accident(s) or incident(s) that have occurred within the workplace and to provide suitable outcomes that can be used to improve the health and safety of the system so that recurrences of the incident(s) can either be reduced or eliminated.
WHO NEEDS TO DO THIS?
This SIRCA investigation is intended for all companies where an accident(s) or incident(s) has occurred within their organisation and an in-depth investigation needs to be made to determine the different route causes that caused the accident(s) or incident(s) and provide outcomes that can be used to improve the health and safety of the system so that recurrences of incidents are either reduced or eliminated.
WHY IS THIS SIRCA INVESTIGATION IMPORTANT?
This SIRCA investigation is important because it determines the different route causes of an accident or incident and provides information on how improvements can be made so that recurrences are reduced or avoided. This provides to an inspector that an incident investigation has been conducted by the organisation when an accident or incident has occurred within its workplace. The investigation of an accident or incident is mandatory under section 9(2) of the General Administrative Regulations and its availability shows that the employer is complying with this legislation in respect of the incident investigation requirements.
VALIDITY
This SIRCA investigation must be conducted by a person who is competent to do so and who has been appointed to carry out the task. The SIRCA investigation must be documented and recorded and must be done within the prescribed period of seven (7) days from the date of the accident or incident and must be finalised as soon as is practically possible.
The document must be reviewed by the relevant health and safety committee and any necessary actions for improvements and prevention of recurrences carried out.
COSTING
This SIRCA investigation is costed per incident investigated. For example, if a user requires 3 (three) incidents to be investigated, the order should be made for the total number of incidents to be investigated:
Three (3) incidents require investigation = 3.
